New Patient Registration Form
New patients to the Shepparton Medical Centre (SMC) will be required to complete the New Patient Registration Form prior to their initial appointment. This will generally be provided when you present to our reception team on the day of your first appointment.
All team members understand their responsibility in regard to your privacy, the confidentiality of your health record and have signed confidentiality agreements. Your patient health record will only be accessed by authorised persons.
The SMC is interested in your views. Please feel free to provide us with feedback.
You can also do this in person at our reception counter, via email (email@example.com) or by phone (03 5823 3100). We aim to work with our patients to respond to feedback provided, if you are not satisfied with our response, you may contact the Office of Victorian Health Services Commissioner on 1800 136 066.
Standard appointments are 15 minutes (one health issue to address).
Longer consultations are available for complex or multiple problems, completion of forms or Care Plans. Please advise the receptionist at time of booking if this applies.
We have a formal agreement with the National Home Doctor Service which means that Shepparton Medical Centre patients can access medical care in their home with the clinic is closed. You can book a home visit by:
- Calling 137 425 OR Booking online: http://www.homedoctor.com.au
- Monday to Friday 6.00pm to 8.00am
- Saturday from 12.00pm
- Sunday and public holidays
Attending your appointment via Video Call
Where appropriate, you can have your consultation online via a video call
Video calling is as convenient as a phone call, with the added value of face-to-face communication.
